Position: Patient Placement Coordinator
Pay Rate: $27.50/hr 
Duration: 13 weeks
Expected Shift: 8am-4:30pm, 40 hrs/week
 
JOB SUMMARY
Required 1 year Hospital experience in care management.
 
The Placement Specialist performs delegated care management functions under the direction of a physician and the registered nurse; documents and prepares the report for delay in treatments/service to registered nurse care managers while assisting with all patient discharge needs. Facilitates communication among members of the Care Management Department, as well as all other departments in the medical center, to ensure appropriate admission and length of stay.
 
ESSENTIAL FUNCTIONS
• Collect pertinent demographic, financial, and clinical data and collaborates with physician and registered nurse to determine appropriate level of care across the continuum; follows established discharge plan based on identified patient needs.
• Assist with coordinating patient discharges placements and arranges post-acute transportation and community services for patients in acute care setting as directed.
• Track actual/pending discharges; follows-up with discharge plan and referrals; orders equipment and supplies; reports discharge barriers and seeks assistance of registered nurse/physician as needed.
• Serves as a liaison among the healthcare team, physician clinics, and patient/family regarding the discharge plan and discharge process.
• Documents communication of information to and receipt of information from post-acute providers/payers as required. Enters authorization numbers and pertinent data into the information system as assigned. Confirms post-acute arrangements with providers, communicating date/time and method of transfer.
• Assist Care Managers with staffing assignments while assisting units with discharge needs.
• Participates in continuous quality improvement activities of the department and Hospital.
• Supports relevant database and generate department reports and statistics as directed.
• Orders supplies/equipment and requests purchasing orders required for patient discharge as directed.
• Provides secretarial and clerical support including faxing, filing and typing
 
JOB SPECIFIC KNOWLEDGE, SKILLS and ABILITIES
• Demonstrated excellent verbal and written communication skills.
• Strong computer and data entry skills (Microsoft Word, Excel preferred).
• Excellent problem solving, conflict resolution and interpersonal skills.
• Able to prioritize and process multiple tasks/responsibilities.
• Knowledge of medical terminology and general medical diagnosis and diagnostic procedures.
• Understanding of Utilization Review and Managed Care and the care management process.
